    Default Mailer Form SSA-455 Disability Update Report (short form)

    so i just received the Mailer Form SSA-455 Disability Update Report. the short 2 page form. i suffer from panic attacks i had been having
    constant attacks for a week when i got this in the mail.. if only i had read what to do online before filling it out. i filled it out correctly.
    but in my panic state i made remarks in the further comments section.. 5. i believe. i blathered on about how my doctor and i are in
    contact online all the time, added the my doctor that has been helping me with my arthritis and spinal stenosis had retired. and didn't say
    my md had taken over for him. ugh. i ended saying i get confused and i was having many panic attack.
    how badly did i mess up? i am still having attacks over this. help

    Hi Don and welcome to Brain Talk.

    In my experience of these periodic reviews, they evaluate what you have said and then either send back a letter saying that there will be no further review at this time or, they ask to see your records.

    I think you will be OK either way.

    calmer now, but at the time i filled it out i was a mess. say on the form it says under Dr appointments you list don't necessarily have to be appointments. i think i put down a couple messages. another thing i did was put down an appointment was upcoming in a day or two. i didn't see my Dr a lot this year but we message frequently.
    i also said my Dr that takes care of my spinal stenosis and arthritis retired, when he actually moved.. then i said i couldn't remember when, as i have a bad memory. wtf was i thinking?! i didn't have to lie i just wrote the first thing that came to mind.. i could have stated after that my MD was taking over for him (he is) but no i lost it and said a bunch of crazyness. good news is my MD is setting me up with new MRI in neck and a specialist. also i asked for a therapist. ya so thats why i have been even more panicked than i need to be.. i'm trying to get my ducks in a row just in case.
    actually i didn't have to say anything it turns out, never comment in that section. ugh
    this is my first review, the short form. it's been 7 years, in 3 years i can retire. i think i'm a low because it's been 7 years since i was approved in the first place.
    thanks for the ear.. you probably can make sense of all this, oh boy.

    Maybe it would help if you would forgive yourselve for any mistakes you think you may have made and just forget about it. It is out of your hands now and we will see what, if anything comes next and deal w that.
